New Patients at Blatt Family Dentistry

The first impression matters.

We start with a comprehensive exam, followed by a consultation because it’s important that you understand your treatment options.

Because your time is valuable to you, and us, some dental treatment might be completed in one day.  However, depending on history, the results of your exam, diagnosis, and treatment plan, further evaluations and additional appointments might be necessary.

To ensure your first visit with us is a seamless experience, please bring the following to your initial appointment:

·         Radiographs (X-rays) if applicable, and your referral slip if seeing our periodontal or endodontic specialists.

·         A list of your current medications and dosages

·         Your dental insurance information, if applicable

It is important and helpful to our dental team that you communicate to them if you have a medical condition that may impact your dental procedure.

Conditions that may affect your procedure include diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.

You should also inform us if you take any medications that may interfere with those we prescribe. These can include heart medications, aspirin, anticoagulant therapy, and many others. If you require medication prior to a dental hygiene cleaning (i.e., antibiotics for pre-med following joint replacement – hip, knee, etc.) please inform our team well in advance.

All patients under the age of 18 must be accompanied by a parent or guardian at their initial examination and consultation appointment.

X-Rays

Please request any recent x-rays or dental images (taken within 12 months) from your previous dental provider or referring dentist. For your convenience, you can have them forwarded to our office by mail or e-mail.

Our office offers state of the art digital imaging that provides the highest standards of diagnostics possible while minimizing patient exposure.

Your dental care is our priority, and we make the process as simple as possible for you. Our fees closely follow the current fee guide recommended by the Ontario Dental Association.

We offer easy billing directly to most insurance carriers, and payment options with Visa, MasterCard, Debit, cheque, or cash.

Monthly payment plans are also available (arranged prior to treatment) for fees not covered by insurance. Our office manager is available to answer any questions about a payment plan.
